Abstract

Background:. Isolated RETROPANCREATIC TUBERCULOUS LYMPHADENITIS presenting as obstructive jaundice is very rare. Till now Limited no of cases reported. Here I am presenting a case report of a 24years male patient presented with obstructive jaundice features with Loss of appetite and No Past history of Tuberculosis. There has been a diagnostic dilemma based on USG abdomen, CECT abdomen, MRI,PET- CT, And EUS and biopsy  whether it is inflammatory or neoplastic. Patient underwent D -lap and biopsy of Retro pancreatic Lymph node and Histopathology suggestive of Retro pancreatic tubercular lymphadenopathy .

Conclusions :Isolated retro pancreatic tuberculous lymphadenitis with obstructive jaundice can mimic as carcinoma. Though it is rare disease this should consider as differential diagnosis in young patient with obstructive jaundice and equivocal imaging findings especially in context of peripancreatic and pancreatic head mass in TB endemic countries like India and avoid major surgical procedure
